We will be providing the COVID-19 vaccine to eligible members of the community on a FIRST-COME, FIRST-SERVE basis. COVID-19 vaccine supply is based on availability from the state's third-party distributors. 


If you have not received any COVID-19 vaccine and are interested in getting the vaccine (first dose), please fill out the form below so we may add you to the waitlist. We will contact you to schedule an appointment once we have available supply of the vaccine.


We will soon begin administering Pfizer boosters.

Those eligible for the Pfizer booster include those who received the second of the two-dose Pfizer vaccine series at least 6 months before and are either:

  • ​65 years or older
  • Residents of long-term care facilities
  • 18-64 years with underlying medical conditions
  • 18-64 years with high institutional or occupational risk (healthcare workers, first responders, teachers and day care staff, grocery workers, and workers in homeless shelters or prisons)
